What sludge is and how it announces itself
Heating water sits in contact with steel radiators, copper pipe and brass fittings for decades. Where oxygen gets in, through repeated topping up, a small leak or an unprotected header, the steel corrodes and the products end up as black iron oxide sludge.
It settles where the flow is slowest, which is the bottom of radiators, and it collects in the boiler where the passages are narrowest. That gives it a specific signature rather than a vague one.
Cold spots at the bottom of several radiators with warm tops is the classic sign. Noisy pipework and a boiler that will not run smoothly point the same way, because restricted flow makes the appliance work against itself. Slow warm-up across the whole house is the third.

What it will not fix
A flush moves debris. It does not correct anything else, and being clear about that prevents disappointment.
One cold radiator while every other one is fine is much more likely to be a closed or seized valve, a stuck thermostatic pin, or a system that has never been balanced. Flushing an entire system to solve a free problem is an expensive way to change nothing.
A weakening pump produces circulation symptoms that closely resemble sludge, and cleaning the system will not restore a pump that can no longer push water round it.
Controls faults do the same. A thermostat sited in the wrong place, or a programmer running short windows, produces uneven heating that no amount of cleaning addresses.

When a system is a poor candidate

That is not a reason to leave a dirty system alone. It is a reason for the assessment to happen before the machine arrives, and for the recommendation to match the condition of the actual system rather than a standard package.
Where a new boiler is being planned, the calculation shifts. A modern heat exchanger has narrower passages and far less tolerance for debris than the appliance it replaces, so cleaning at the point of replacement protects both the appliance and the warranty.

Making the result last
A flush on its own is temporary. The corrosion that produced the sludge resumes the moment the system is refilled unless something changes.
Inhibitor goes back in at the correct concentration. Draining a system removes the inhibitor along with the water, so refilling without replacing it leaves the system unprotected from that day forward. It is a small cost with a large consequence.
A magnetic filter is the other half. It catches iron oxide as it forms and gives every future service somewhere to remove it from, which is why fitting one on a system that has just needed cleaning is close to essential rather than optional.
Then find whatever was letting oxygen in. Pressure that keeps falling and gets topped up is the most common cause, and each refill brings fresh oxygenated water into a system you have just paid to clean. Balancing checks afterwards make sure the improvement is even across the house rather than concentrated near the boiler.
